What's it like to be a Care Coordinator?
Care Coordinators facilitate support and services for elderly people and people with disabilities. They work closely with patients and families to plan care programs for people in community or in-home settings.
Tasks and duties
- Developing goals and individual care plans in collaboration with patients and families.
- Facilitating access to clinical services and health practitioners.
- Maintaining clinical records and writing reports.
- Tracking performance of patient care plans.
- Ensuring compliance with relevant legislation and guidelines.
- Rostering clinical services and staff members.
- Advocating for patients and clients.
Care Coordinators need to be empathetic and have some knowledge of the ethical and legal concerns in their industry. They typically work in the aged care and health care industries.

A relevant qualification and experience in a community services role is usually required to become a Care Coordinator. In some cases, an equivalent level of experience may substitute for a qualification. Up-to-date immunisations are required and you may need a current driver’s licence.
- 1.Complete a Certificate III in Individual Support (CHC33015). This usually takes one year and involves a mix of theoretical learning and work placement.
- 2.Ensure you have up-to-date immunisations as required and complete certifications and checks such as a First Aid Certificate, Working with Children Check and/or a Police Check.
- 3.Build experience in individual care, in an entry level role such as Disability Support Worker or Aged Care Worker.
- 4.Consider completing further study in your chosen field, such as a Certificate IV in Ageing Support (CHC43015) or Certificate IV in Disability (CHC43115).
